GENERAL INFORMATION
Colonial Center is a full-service nursing facility offering rehabilitative care, long-term care, and skilled nursing. Nursing care is provided for those recovering from illness, injury, or those requiring 24-hour care. Our specialized approach is designed to help each resident achieve the highest level of independence possible. We deliver the most comprehensive care available by providing for the physical, emotional, spiritual, and social needs of our residents. Our nursing community is Medicare/Medicaid certified. Colonial Center is an active member of the Wisconsin Association of Homes and Services for the Aged.

The goal of restorative therapy is to enhance the life of each resident by customizing a personal program to bring them to the fullest level of recovery. Our philosophy advocates treating the whole person and affirming the dignity of each individual. Our programs include therapies designed to maintain functioning and promote the highest level of independence possible.

MEMORY CARE
Earle's Village Memory Care Neighborhood was created to serve those residents facing Alzheimer's Disease, Dementia, or other memory challenges. It's a place where residents feel safe, secure, and well loved.

Colonial Center is there for those who are recovering from illnesses, accidents, or surgery. Since our goal is to return residents to their home setting whenever possible, we offer a full array of in-patient rehabilitation therapies. Skilled therapists help residents regain strength, relearn skills, and become more confident and independent throughout their daily lives. When returning home, our licensed professionals will work closely with health care providers and community resources to ensure supportive services are in place.
